Have you received clearance from a doctor to be involved in an exercise program?
Yes
Are you working with any health practitioners? If yes, what is their name and do you give permission for me to contact them if required?
Yes - Dr Sarah Lovibond at Ochre Medical
Centre John Street, and Hannah Calvert at the Pelvic Studio.
Do you menstruate on a regular basis (outside of pregnancy) and how long is a typical cycle? Any concerns around your menstrual cycle? Do you track your cycle?
Yes I track my period and it’s a very regular 28 day cycle. No concerns.
How many children/pregnancies have you had? List DOB and type of birth (vaginal, planned c-section, unplanned c-section, D&C)
Two physiological/vaginal births. Edie - 7/8/23 and Jem 4/6/26
For your most recent birth, please provide any relevant information (length of labour, pushing phase, tears/episiotomy, assisted delivery, stitches, scar pain/sensation, etc.)
2.5 hours. Intense with foetal ejection reflex. 1st degree tear and some grazing - no stitches.
For any previous births, please share any details you believe relevant
Asymptomatic prolapse from first birth.
Are you currently breastfeeding?
Yes
Has your menstrual cycle returned?
Irregularly
Have you had your 6 week check and received clearance for exercise?
Yes
What is your exercise history? What were you doing previously and what have you done in the last 6 months?
Minimal
Planned exercise during pregnancy. I’ve resumed my yoga practice (two vinyasa classes a week), do relaxed walks and keep up with a three year old.
What are your goals / reasons for training throughout your pregnancy / returning to exercise?
Developing strength - particularly in my core and back for carrying and lifting my children, to have energy to keep up with them, and to sustain a healthy mental headspace.
Do you have any concerns about exercising / anything important for me to know? (e.g. previous injuries, pain, fatigue)
I generally avoid jumping or high-impact movements as I’m prone to sore knees.
Have you seen, or do you see, a Women's Health Physio? If yes, what did you go for and what was the result?
I had a few appointments during my second pregnancy, mostly to monitor my prolapse. No concerns.
If you are currently seeing a Women's Health Physio, what is their name and do you consent to me making contact with them?
Hannah Calvert at Pelvic Studio - yes.
Do any of the following apply?
I have noticed a bulging from front or back passage, or am aware I have a prolapse
Have you ever experienced back or pelvic pain?
During my second pregnancy I experienced significant back and pelvic pain but that has stopped since I’ve given birth.
Have you been assessed for Diastasis Recti (abdominal separation)? If so, when and what was the result?
Yes - no indicators.
How did you hear about us?
Saltbush and friends.
Do you agree for photographs to be taken during sessions and used on social media for future promotional literature and/or course content?
No

Sarah Lovibond - GP (Ochre Medical Centre John Street)
Hannah Calvert - Pelvic floor physio (Pelvic Studio)
Yes to contact.
Do you menstruate on a regular basis (outside of pregnancy) and how long is a typical cycle? Any concerns around your menstrual cycle? Do you track your cycle?
Not yet menstruating since giving birth 8 weeks ago. Previously very regular cycle - usually 28 days and yes I tracked it.
How many children/pregnancies have you had? List DOB and type of birth (vaginal, planned c-section, unplanned c-section, D&C)
Two children/pregnancies.
Edie - 7/8/2023
Jemima - 4/6/2026
Both were physiological vaginal births without instruments.
For your most recent birth, please provide any relevant information (length of labour, pushing phase, tears/episiotomy, assisted delivery, stitches, scar pain/sensation, etc.)
It was a very short and intense labour, lasting 2.5 hours from first contraction to baby on my chest. I may have had the foetal ejection reflex as I was unaware of pushing. Slight grazing and one 1st degree tear that didn’t require any stitches. Physical recovery was very quick.
For any previous births, please share any details you believe relevant
2nd degree tearing that required stitching, healed well despite some initial scar tissue. Completely healed now.
Developed a prolapse of both front and back vaginal walls - always asymptomatic, only diagnosed by feeling and looking. I usually wear a ring pessary.
Are you currently breastfeeding?
Yes
Has your menstrual cycle returned?
No
Have you had your 6 week check and received clearance for exercise?
Yes
What is your exercise history? What were you doing previously and what have you done in the last 6 months?
Pregnancy yoga and keeping up with a toddler during pregnancy.
Generally my preferred movement is walking and doing three yoga classes a week (a mix of vinyasa and yin).
What are your goals / focus returning to exercise?
To feel strong, flexible, prevent injury and feel energetic with my daughters.
Do you have any concerns about exercising / anything important for me to know? (e.g. previous injuries, pain, fatigue)
Building core strength in a safe way considering I’m still early postpartum.
Is there anything particular you wish to learn / work with me on?
Alignment and strength that supports lifting / carrying children.
Have you seen, or do you see, a Women's Health Physio? If yes, what did you go for and what was the result?
I first saw a pelvic floor physio after my first birth as I suspected a prolapse. She supported me with this, and I returned to her intermittently to check my pessary and then again during my second pregnancy, particularly regarding pelvic girdle pain.
If you are currently seeing a Women's Health Physio, what is their name and do you consent to me making contact with them?
Yes - Hannah Calvert at The Pelvic Studio
Do any of the following apply?
I have noticed a bulging from front or back passage, or am aware I have a prolapse
Have you ever experienced back or pelvic pain?
Yes - during my second pregnancy. I’ve not experienced it since giving birth.
Have you been assessed for Diastasis Recti (abdominal separation)? If so, when and what was the result?
I haven’t been assessed since recently giving birth.
